MATERNAL ENVIRONMENTAL EXPOSURE TO METALS(LOIDES) AND THE DEVELOPMENT OF GESTATIONAL HYPERTENSIVE DISORDERS IN RESIDENTS OF RONDONIA

Introduction: Gestational hypertensive disorders (GHD) contribute significantly to adverse obstetric and neonatal outcomes; although their etiopathogenic mechanisms are not precise, environmental exposure to metals (loids) appears to be associated with systemic repercussions that influence their development. Objective: To investigate the association between maternal environmental exposure to aluminum, arsenic, barium, cadmium, lead, strontium, and mercury and the development of hypertensive disorders in a high-risk pregnancy maternity hospital in the state of Rondonia. Method: This is an observational-analytical case-control study with 77 postpartum women with GHD and 154 postpartum women without the disease. Data were collected using a specific form and by consulting electronic medical records. Breast milk samples were collected to perform metal (loid) analysis. The concentrations of total mercury (HgT) in breast milk were determined by cold vapor atomic absorption spectroscopy with a flow injection system and the concentrations of other metals (loid) by optical emission spectrometry with inductively coupled plasma. Descriptive statistics, Pearson's chi-square test, Fisher's exact test and Mann-Whitney U test were performed and complemented by Principal Component Analysis (PCA); binary and conditional logistic regression models were adjusted to evaluate the associated factors and the influence of metal concentrations on the condition studied. Results: Women with HDP were older, had a higher body mass index (BMI), were more obese and reported using chemical hair products when compared to normotensive women. The case group had higher concentrations of strontium and lower concentrations of barium in breast milk. After adjusting the models, exposure to metals was not associated with HDG, whereas the use of chemical products for straightening or hair dye and pre-gestational BMI greater than 25 kg/m2 appear to be determinants for the disease. Conclusion: Although the data do not express cause-effect between environmental exposure to metals (loid) and the development of HDG, they confirm pre-gestational BMI greater than 25 kg/m2 as a risk factor and demonstrate that the use of straightening products and/or hair dye appears to contribute to the disease. Therefore, alerting health professionals and women about the risks of using certain products with compounds that have been proven to be harmful to health, encouraging the implementation of standardized care protocols for identifying and managing obstetric risks, adequate management in the prevention, diagnosis, and treatment of HDG variants, and longitudinal follow-up of women's care, and encouraging studies that seek to analyze environmental risks and contribute to the formulation of public policies related to environmental quality and health can be collaborative strategies for improving maternal health indicators.
