ABSTRACT

This study is based on the recognition that regional economic development is closely linked to the quality of human capital and the reduction of social and territorial inequalities—dimensions in which public health plays a central role. In this context, the research seeks to investigate how the application of risk management practices, grounded in the COSO framework, can influence organizational performance and process improvement in public health policies in the State of Rondonia, Brazil. The general objective is to analyze the impacts and challenges of risk management within the strategic axis of the state-level Unified Health System (SUS), focusing on identifying the foundations of risk management practices adopted in Rondonia, measuring the maturity level of strategic management practices in the State Health Department (SESAU-RO), and assessing the implications of these practices for process performance in the health sector. The study adopts a pragmatic paradigm, with a predominantly qualitative approach supported by quantitative data, using action research as its methodological strategy. As a result, a ten-step risk management cycle was proposed and validated, applied both to the macro process of complying with the constitutional health funding index and to the Laboratorio Estadual de Patologia e Analises Clinicas (LEPAC-RO) during a R$14 million laboratory automation procurement process. The applicability and effectiveness of the proposed cycle were demonstrated, leading to its subsequent adoption by the Court of Accounts of the State of Rondonia for use in its internal processes. The maturity assessment of SESAU-RO’s internal controls reached an overall score of 67.69%, classified as an intermediate level of adherence to the COSO framework. The “control environment” component showed the lowest adherence (57.73%), while “monitoring” reached the highest score (85.94%), with the remaining components ranging between 61% and 72%. These partial results indicate progress in the structuring of internal controls and in the risk governance capacity of the state’s health system. The study continues with the evaluation of the strategic implications of this risk management approach (Objective III), aiming to integrate empirical analysis of normative maturity with practical intervention in the implementation of risk management practices to enhance the performance of public health processes in Rondonia.
