MATERNAL ENVIRONMENTAL EXPOSURE TO METALS(LOIDES) AND THE DEVELOPMENT OF GESTATIONAL HYPERTENSIVE DISORDERS IN RESIDENTS OF RONDÔNIA

Introduction: Gestational hypertensive disorders (GHD) strongly contribute to adverse
obstetric and neonatal outcomes; although its etiopathogenic mechanisms are not exact,
environmental exposure to metals (loids) seems to be associated with systemic repercussions
that influence its development. Objective: To investigate the potential effects of maternal
environmental exposure to aluminum, arsenic, barium, cadmium, lead, strontium and mercury
on the development of hypertensive disorders in a high-risk pregnancy maternity hospital in
the State of Rondônia. Method: Observational-analytical case-control study with 77
postpartum women with HDP and 154 healthy postpartum women. Data were collected using
a specific form and consultation of electronic medical records. Breast milk samples were
collected to perform biomonitoring of the participants. The concentrations of total mercury
(HgT) in breast milk were determined by cold vapor atomic absorption spectroscopy with a
flow injection system and of other metals (loids) by optical emission spectrometry with
inductively coupled plasma. Descriptive statistics and chi-square tests, student t tests for
independent samples and Mann-Whitne U tests were performed; the associated factors and the
dependency relationship will be investigated using logistic regression. Partial results:
Women with DHG, when compared to women without the outcome, had a higher mean age
(x̄case = 28.03 ± 6.88; x̄control = 24.81 ± 6.29; t test (0.05;229) = 3.544; p<0.05), as well as
higher frequencies of obesity (case = 22.1%; control = 4.5%; 11.63%; control = 0; The
concentration of total mercury (HgT) in breast milk in the case group was higher (Md: 0.8698
IQR: 0.9989) than in the group of healthy women (Md: 0.6607 IQR: 0.8962), a statistically
significant difference (U (0.05;229) = 4348; p = 0.022); the control group (77.17%) had a
higher proportion of HgT concentrations below the limits recommended by the World Health
Organization in relation to the case group (71.62%), statistically significant (X2 =7.9037; p =
0.019) . In relation to the other elements investigated, lead and strontium presented higher
concentrations in participants with DHG, but without statistical significance. Partial
considerations: although preliminary data are not able to express cause-effect between
environmental exposure to metals (loids) and the development of DHG, it is observed that
women with the outcome appear to contain higher concentrations of total mercury in breast
milk.
