PROSPECTIVE STUDY OF THE STATE OF HEALTH OF A RIVERINE POPULATION OF THE BRAZILIAN AMAZON

This study aims to describe the predominance of cardiometabolic syndromes, overweight and obesity in a riverine population in the Brazilian municipality of Humaitá, and to assess the main risk factors for these conditions while taking into consideration the regional development that occurred in recent years. It is a longitudinal, ambidirectional, double-blind study associated with a descriptive and analytical approach of a census dated from the period between 2010 and 2017. The subjects in this study are traditional riverside residents of a semi-isolated outskirt area of Humaitá. The choice of this study population was based off data collected systematically since 2010, regarding the socioeconomic status, nutritional profile, clinical-laboratory data and anthropometric parameters. The results show that there was a statistically significant weight gain and accumulation of body fat, accompanied by an increase in blood pressure, high levels of cholesterol and triglycerides, decreased levels of physical activity and impaired life quality. When comparing the results between the years of 2010 and 2017 it is possible to confirm the presence of nutritional transition. Given the lipophilic nature of methylmercury present in the Amazonian riverine population, the human contamination by this metal was analyzed through serum and urine samples. We found a positive correlation between the percentage of body fat and serum levels of mercury (p <0.05). Although there was a decline in the population’s consumption of fish, serum mercury levels remain high in most of the locals and it’s correlated to the body fat percentage. We believe our study to be the first to bring light to the evidence that there is a statistically significant relation between obesity and levels of mercury contamination in humans. In conclusion, an increase in economic power followed by changes in the traditional lifestyle of the Amazonian riverines, such as NT and sedentary lifestyle, resulted in overweight, obesity, hypertension, fat accumulation and high serum levels of mercury correlated with the body fat percentage. The quality of life for the residents of Puruzinho Lake was affected by NCDs as well as changes in lifestyles. This included clinical signs and symptoms potentially related to hypertension, elevated levels of triglycerides, limitations of daily activities, and some dependence on electricity and in the markets.
