Air pollution is an emerging public health issue on a global scale due to the trade-offs to both human health and the environment. Burning and forest fires are characteristic of the main sources of elimination of the quality of the Brazilian Amazon, promoting the emission of emissions such as fine particulate matter (PM2.5), carbon monoxide (CO), nitrogen oxides (NOx) and volatile organic compounds (VOCs), from the combustion of biomass. Among atmospheric toxins, PM2.5 appears in scientific evidence as a risk factor associated with respiratory diseases and increased premature mortality in vulnerable populations, especially in elderly people, with a greater risk of hospitalizations and mortality. This study was investigated by the association between PM2.5 and adverse stages that resulted in hospitalizations in elderly people between July 2021 and July 2024 in the municipalities of Porto Velho and Cacoal. The study design was ecological and had a quantitative approach. Information on respiratory hospitalizations was collected from the public domain Hospital Information System of the Ministry of Health. Annual and daily concentrations of PM2.5 from low-cost sensors (PurpleAir-II-SD) were determined using the API processed in R software, version 4.5.0. The results of the association between PM2.5 and respiratory outcomes were presented by Relative Risk (RR) with a 95% confidence interval (CI) for an increase of 10 μg/m3, using a generalized additive model adjusted with quasi-poisson regression and nonlinear distributed lag model (DLNM). The average concentrations of PM2.5 per location throughout the study period ranged from 1 to 57.2 (µg/m3) for Cacoal, 1.9 to 190.4 (µg/m3) measured by the residential sensor and 1 to 243.5 (µg/m3) in the public sensor, both in the municipality of Porto Velho. An immediate effect and strong association was found between the increased risk of the outcome on the day of exposure (lag0) for lower airway infections (URTIs) in Porto Velho, with RR=2.5 (95%CI), statistically significant up to lag 4 (95%CI) and a gradual reduction in the effect RR=1.1 at lag 7. And in Cacoal, the RR=1.5 (95%CI), indicated an increase in the outcome of Chronic Obstructive Pulmonary Diseases (COPD) at lag 3, indicating that the effects of exposure to the pollutant are delayed, showing a delayed association between exposure and the increase in the outcome. Low-cost sensors are indicative and viable devices, when associated with other air quality monitoring technologies they contribute to a greater geographic coverage, assisting in public policies to control air pollution, especially during critical pollution episodes in low- and middle-income countries.
